April 2, 2019 from COR Detroit: .
WATER: Any unflavored, spring or distilled or purified bottled water with no additives listed may be used. (Note: 3 and 5 gallon reusable containers should not be used for Pesach.) Water with added minerals (such as magnesium, potassium, calcium and sodium compounds), is generally acceptable if no flavors or vitamins are listed; e.g., Dasani and Nestle Pure Life brands of unflavored purified water. However, some brands, e.g., Kirkland Signature brand Purified Water with minerals added, list calcium citrate, which may be derived from chometz, and are not recommended

March 30, 2021 from the Ira Axelrod, a Boston based food distributor:
"It has come to our attention that a large number of bottles of Taam Tuv red horseradish, supposedly intended for Passover use, did not have the appropriate Passover markings on the jars. It is possible that it was only a printing or labelling error. But it is also possible that jars NOT meant for Passover got mixed in with the Passover batch. Apparently, a significant number of these jars wound up in the Boston area."
"Until the situation is fully investigated and is concluded to everyone's satisfaction, it has been recommended that, if you have any of these jars, you refrain from using them."
From the OK in response to a question from the editor: Any chometz in the formula would be botul.
April 1, 2021 from the MK:
The MK wishes to inform the public that this year changes have been made at IGA 4885 VAN HORNE with regards to them displaying their passover products.
Unfortunately, a number of people have inadvertently purchased items which are not suitable for passover.
We remind the public once again that prior to purchasing a passover product, please ensure the product is indeed kosher for passover.
March 26, 2018: From the Beth Din of Johannesburg:
It has just come to our attention that the supplier sent non-Kosher for Pesach KEGLEVICH VODKA together with the Kosher for Pesach product.
If you have bought this product from any outlet, please make sure that it specifically says Kosher for Pesach - כשר לפסח (in Hebrew).
